It's discretionary whether that side effect is worse than the condition / disease it's seeking to alleviate. Caffeine may be the best in this equation.

In this instance, it's vanity. Even if they ran a commercial right now that said you have a high probability of losing a limb, I bet you'd still have people signing up in droves, if it helped them lose weight quickly.
But the people in them look so happy and totally casual while the side effects of death are being listed!
