It was reviewed and overturned. It's reviewed to see if it meets the elements of targeting. If it's questionable it gets called simply to review it and be sure. If the DB had launched or used the crown of his helmet the call would have stood.
The ref's not making the final judgement. The review booth is. The ref simply reports the review booth's determination.
