No, they don't have to have anti-trust exemption to have collective bargaining. MLB has an exclusion, but it is judicial in nature not legislative. The other three leagues don't have exemptions from the Sherman Act. Without going into detail, they basically have been able to argue when it comes to their relationship with their players, while their behavior is monopolistic, and possibly violative of Sherman, there are exceptions they have been able to prove to stay out of trouble.
